Pregnancy is a crucial period of 9 months, usually 38–40 weeks. In routine, all women confirm their pregnancy, by UPT or a pregnancy kit when they miss their menstrual cycle/period. Counting of pregnancy months usually starts from that missed month, but the gestation period has actually started from the last menstrual cycle (on the day of ovulation). So overall, gestation is a period of around 40 weeks in a normal pregnancy.

In the last menstrual cycle, fertilization occurs on the day of ovulation, which usually occurs at the 14th day of the 28-day menstrual cycle. After that, changes start to occur in a woman’s body.

When do these signs and symptoms appear?

  • 1 to 2 weeks after conception: Mild cramps in lower abdomen, which may be accompanied by implantation bleeding.
  • 3 to 4 weeks after conception: Absence of period, fatigue, noticeable breast tenderness.
  • 5 to 6 weeks after conception: Nausea and vomiting, frequent urination, heightened sensitivity to smells.

A few symptoms are discussed below which occur in pregnancy:

1) Morning Sickness

Earliest symptom which starts to occur after pregnancy is conceived. Progesterone is the hormone responsible for morning sickness. This hormone leads to relaxation of the lower esophageal sphincter, which leads to reflux of stomach contents into the food pipe.

Morning sickness includes:

  • Nausea
  • Vomiting
  • Discomfort in stomach

It is a physiological symptom which occurs in all pregnancy. This symptom is more likely to occur during first trimester of pregnancy. This symptom is resolve by 16–20 weeks of pregnancy but sometime may extend up to 32 weeks of pregnancy.

In this condition, a person is more intolerant toward liquids.

Warning-If someone is having severe vomiting with pain and discomfort in stomach & weight loss is also occurring with all these symptoms, then you should consult a doctor.

2) Missed Period

If the fertilization has occurred, then the menses onward are not going to happen in a woman.

A missed period is one of the symptoms which points towards pregnancy. After missing a period, one should undergo UPT (Urine Pregnancy Test) or pregnancy kit test to provisionally confirm the pregnancy.

For final confirmation of pregnancy, one should undergo through ultrasound or a specific pregnancy test by consultation of a doctor.

3) Breast Changes (Swollen & Tenderness)

(i)  Size and weight of breast increase due to:

  • Increased fat (effect of insulin)
  • Increased alveoli (effect of progesterone, estrogen, prolactin)
  • Increased ducts (effect of estrogen)

Alveoli & ducts are part of breast tissue.

(ii) Hyperpigmentation of Breast Occurs (Especially around areola – area around nipple)

Breast changes occur in the first trimester of pregnancy. During this period, the breast increases in size which leads them to appear swollen. Tenderness also occurs during this period, which diminishes after the 3rd month.

4) Frequent Urine Flow (Renal Changes)

Usually, the size of the kidney increases by 1 cm during pregnancy. Physiological changes also occur in the renal system during pregnancy:

  • Increase blood flow through kidneys
  • Increase glomerular filtration rate
  • Increase filtration capacity of kidneys

These changes lead to frequent urine flow during pregnancy. Sometimes, these changes of the renal system may lead to urinary tract infection (UTI) in pregnancy. The most common cause of UTI is E. coli.

5) Fatigue & Weakness

Fatigueness and weakness usually occur in the prior phase of pregnancy.

These changes occur because of increase blood circulation and other blood profile changes that occur during pregnancy.

These symptoms occur during the 1st trimester of pregnancy and regress after the 3rd month of pregnancy.

Other causes:

  • Dietary factors (low-fiber diet)
  • Reduced physical activity
  • Uterine pressure (during last trimester)

6) Bloating & Constipation

These are the early GIT (Gastro-intestinal tract) symptoms which appear during pregnancy. These can occur due to many reasons, but the main reason is hormonal changes.

Progesterone is a hormone which relaxes the smooth muscles of the GIT.

These lead to reduced motility of GIT content and cause constipation & bloating.

7) Food Aversion & Cravings

Elevated levels of hCG, estrogen (hormones) can alter taste & smell receptors, which can cause food aversion of specific foods which are sour, strong odor, or oily-based.

Generally, food items include strong-smelling items like:

  • fish & meat
  • eggs
  • onions
  • garlic
  • fried or greasy foods
  • coffee & alcohol

8. Headache & dizziness

These symptoms are commonly occurred during 1st & 3rd trimester. Common causes of these symptoms are i.e. hormonal changes, stress & fatigue, dehydration, low blood sugar, caffeine withdrawal, sinus congestion etc. Standing up too quickly can lead to dizziness due to postural hypotension.

9. Cramps

Pain in lower abdomen may occur during early stages of pregnancy because of increased blood supply & nutrient supply to that area because of growing embryo which may lead to cramps in lower abdomen.

10. Vaginal changes (discharge, discoloration)

During pregnancy bluish discoloration of vagina occurs k/a Chadwick sign or Jacquemier’s sign. PH of vagina also decrease during pregnancy which lead to acidic nature of vagina which prevent the growth of pathogenic bacteria/vaginal infection.

11. Cervical changes (infection):

Secretion from the cervix becomes thick and form a plug around cervical mouth. It is a body’s natural defence mechanism to prevent infection.

12. Shortness of breath:

It is a common symptom which can be felt during whole pregnancy but cause is different for different trimesters.

During 1st trimester because of hormonal changes, respiratory centre in brain is stimulated which make person feel shortness of breath.

During 2nd & 3rd trimester, because of increase in size in uterus size, diaphragm pushed upward which reduce lung capacity. Therefore, person can feel shortness of breath.

13. Acute sense of smell:

Heightened smelly sense is because of increased level of HCG & estrogen hormone. Strong smelly food can lead to nausea & vomiting; therefore, intolerance occur against smelly odor foods.

14.Backpain:

Relaxin is a hormone which lead to loosen up of ligament & joints in body to prepare body for the childbirth which affects the stability of backbone hence backpain occurs.

15. Mood swings:

Mood swings occur because of hormonal changes, fatigue & emotional distress. Constant tiredness can make hard to manage emotions for pregnant women.

Case Study: Early Symptoms Leading to Pregnancy Confirmation

A 29-year-old woman with regular 28-day menstrual cycles reported a missed period of 5 days along with mild lower abdominal cramps, breast tenderness, and increasing morning nausea. She also recalled light brown spotting about 8–9 days after ovulation, which she initially assumed was premenstrual spotting. A home urine pregnancy test performed after the missed period showed a positive result. Serum beta-hCG levels confirmed early pregnancy, and a 6-week ultrasound revealed an intrauterine gestational sac with fetal cardiac activity. Her symptoms were consistent with normal early pregnancy changes, and no signs of ectopic pregnancy were found. This case highlights that a missed period combined with mild cramps, breast changes, and nausea strongly suggests early pregnancy and should be confirmed with appropriate testing.

Research Insight: Hormonal Changes Behind Early Pregnancy Symptoms

According to the American College of Obstetricians and Gynaecologists, human chorionic gonadotropin (hCG) becomes detectable in maternal blood approximately 8–11 days after conception and is the key hormone responsible for confirming pregnancy. Rising levels of hCG and progesterone contribute to common early pregnancy symptoms such as nausea, breast tenderness, fatigue, and frequent urination. The organization also emphasizes that urine pregnancy tests are most reliable after the first missed period, as testing too early can result in false-negative outcomes due to low hormone levels.

How reliable are early pregnancy signs

On the basis of reliability these early signs are classified as-

1. Presumptive signs- these symptoms can also called as uncertain symptoms as well. These can also occur in other common condition not specific for pregnancy. e.g. nausea, fatigue, breast tenderness, frequent tenderness.

2. Probable signs- presence of these sign means there are more likely chance of pregnancy. these signs strongly suggest pregnancy but not 100%. E.g. missed period, positive urine or blood test.

3. Positive signs- signs that can only be caused by pregnancy. e.g. fetal heartbeat and movements which only detect by ultrasound.

Time when you should go through test and see a doctor

It’s a urine pregnancy which is performed for the provisional confirmation of the pregnancy but it’s performed from the day your periods are due. This test may not be reliable if performed too early can give false negative results as well. So, to prevent this false result, perform test at an adequate time.

After confirmation of the pregnancy at home by positive urine pregnancy test, a medical confirmation is advisable, mostly done between 6th to 8th week of pregnancy. this can give confirmational results about pregnancy status of any women.

When to seek urgent medical attention

If you are experience any of the following symptoms then it’s time to consult your doctor urgently. These symptoms include-

  • heavy vaginal bleeding
  • severe pain on one side of abdomen
  • severe dizziness or fainting
  • severe vomiting that cannot keep liquids down

FAQs-

  1. Q. Can someone have these symptoms just a few days after sexual intercourse?

    Ans usually no, as it is explained in this blog that symptoms are going to start to appears at least 1 to 2 weeks after conception. By this time implantation occurs and spikes of hormones seen after implantation that led to appearance of these symptoms.

  2. Q. Does absence of symptoms mean that something is wrong?

    Ans absolutely not, there are many women who only have very mild or not at all any early sign and symptoms of pregnancy and still have a very healthy pregnancy and delivers a healthy baby.

  3. Q. What is implantation bleeding?

    Ans, it refers to small amount of blood spotting that occur when fertilised egg implanted itself into uterine wall.

  4. How implantation bleeding differs from normal periods?

    Ans implantation bleeding is usually-
    Very light means it’s not like a blood flow but just a spotting.
    Colour is usually pink or brownish not bright red colour which appear in normal period.
    Significantly it occurs for shorter time of duration (few hours to couple of days) than menstrual cycle.
